program for the purpose of rebalancing of your mineral status.

You understand that HTMA interpretation - mineral balancing is not intended to be a diagnosis, a treatment, or a

cure of any disease or health condition. It is also not intended as a substitute for regular medical care and that

You are encouraged to seek a second opinion from a medical care provider. Any information is considered

general and educational information only.

You understand that under NO circumstances is medication to be discontinued without consulting first with the

prescribing medical provider. You are encouraged to see your medical care provider for diagnosis and treatment

of any medical conditions You may have. You should discuss any diet, supplement, exercise, or other lifestyle

change with your medical provider.

You understand that HTMA interpretation - mineral balancing is to be used at your own risk. You understand that

if You use the suggestions recommended to rebalance your mineral status, that You are strongly encouraged to

order a mineral hair analysis re-test in 3-4 months to determine if the program needs to be modified.

All information provided by You is kept strictly confidential in accordance with all applicable laws. None of your

information will ever be shared or discussed with any third party without express written consent.
